POLYCARE develops ICT solutions to enhance integrated care for chronic patients, enabling them to manage their health at home. The project focuses on providing personalized apps and devices that facilitate communication between patients and healthcare services, improving quality of life and reducing hospital visits.
Nowadays, chronic diseases and co-morbidities have to be managed during a long period of time among health and social care systems, thus a big amount of budget and resources should be destined to them.
