ProACT aims to enhance self-management for Europe's 50 million patients with multiple chronic conditions by integrating various technologies into a home-based care system. It focuses on data analytics to improve health outcomes and quality of life through personalized care networks and coordinated support.
ProACT targets Europe’s 50 million multimorbid patients to proactively self-manage and offset the EU’s annual €700billion cost of chronic disease management.
