This project develops a digital health platform, Risk-Enhanced Community Care after Discharge (REConneCteD), to support children recovering from severe infections in Sub-Saharan Africa. It aims to improve post-discharge care through community health workers, enhancing continuity of care.
Children returning home after hospitalization for severe infectious illnesses suffer high mortality rates, especially in sub-Saharan Africa (SSA).
After returning home, continued engagement with the health system can improve recovery and reduce post-discharge deaths.
