The AFFIRMO project addresses the challenges of managing atrial fibrillation in older patients with multiple health conditions. By identifying disease clusters and developing a patient-centered care framework, it aims to improve clinical management and optimize healthcare delivery for this population.
Multimorbidity is a common condition in older age, and can substantially influence individuals’ health and quality of life, making management more difficult.
