The ESCAPE project aims to improve care for elderly patients with multiple health conditions by integrating treatment for mental health issues. It utilizes a collaborative care approach and advanced technology to customize patient care and enhance quality of life.
ESCAPE will rethink current practice for treating multimorbidity in elderly patients and break down health care silos that impede integrated care and lead to adverse outcomes.
